Please complete the necessary form(s) and release(s) prior to your first visit. 

Patient Portal

You will be receiving a confirmation email and an invitation to our patient portal once your appointment has been made.  Please sign up for the portal right away as this is your access point to your new patient paperwork, as well as your future access to your visit notes, appointment times and the best way to communicate with your doctor, and pay any future bills.  The patient portal is HIPPA compliant, very easy to use and all forms will be filled out electronically.

New Patient Paperwork

It is very important to fill out your intake forms and sign your releases within 24 hours to confirm your appointment.  Your provider will have access to these forms immediately once completed.  If you are not able to complete the necessary forms ahead of time, please call us to let us know and plan on coming in at least 30 minutes early to complete them in the office.  You will miss valuable time with your provider if you don’t fill them out before your visit.

Even if you are not scheduled for an acupuncture appointment, it is a good idea to sign and have this agreement on file in case your provider should refer you to one of our acupuncturists.  If you do not wish to sign this form, please let our receptionist know.

Please take a moment to review our release forms. Once you have read these documents, please sign the electronic copy either through the patient portal, or at the kiosk when you check in at reception.
